I need to get a doctors certificate sorted out for dd. Can someone please tell me does it need to be a letter written by the doctor or is there a standard form he can just sign instead? Have looked on the local authority child performance licences page and there is a medical declaration form on there but it doesn't seem to have a piece for the doctor to sign, just a declaration as a parent. Can anyone help, I'm confused!

In our doctors they have a standard letter and it justs gets stamped and doctor sign it. Cost £10 (thats the worst part) and it last 6 months thankfully. Good luck with the job.

We managed to get an undated Doctor's letter & thankfully, he didn't charge us!

Thanks, we haven't got a job yet but understood from the agent that we must have one in anticipation...I should have sorted it out before now. I think I will pop to the doctors in the morning. When I called they told me they couldn't tell me how much it would cost as it was private work and up to the doctor to decide. I must admit I fear the worst with our doctors!

Fudge Id say get the doctors note signed.. My LEA does not require a doctors note , a parental declaration is enough, but Abacus pointed out to me that if i sign the parental declaration, & don't get a doctors note, & my daughter was ill , etc then I as the parent who signed that she was of good health, would be liable for any costs incurred by the production company.. Something Id rather not be.. If your doctors don't have a standard letter let me know & ill try & find a blank copy of mine for your doctor to sign, & PM it to you!!! It costs me about £12 & doctors surgeries differ vastly in price..

Thanks that makes sense then, our LEA must be the same. I will see what they say at the doctors tomorrow. I know when I had to have a letter for work a couple of years ago they charged me around £50 + VAT so that's what I'm worried about. Just annoying that they can't/won't tell me how much it will cost.

i just made a routine appointment,didn't tell the receptionist what it was for and had the letter all ready for the doctor to sign.
he was really friendly and signed it for free.....maybe you could try that approach...has worked twice for me now!!!

My doctors charge £35 have heard of parents being charged more just got ds done but doctor has not put a date am leaving it tell he gets a job then will try and get the doctors to date it from that date
