For Spotlight Young Performers, do the majority of casting breakdowns get sent to a select number of agents, or do they tend to go to all of them?
Also, will my agent fill in the form for me, or do I need to get one and fill it in for myself?

Not sure about how it works but I have one agent that completes it for us and one that doesn't .....I'm sure others will be able to help you though on your other question.

I think that - like the adult performers - it will depend on the job. Commercials are highly likely to get sent to all agents. For drama it may well be that some CDs will go to some agents first - and then open it up a bit if they don't find what they want. Of course, just like adult casting, there will also be occasions where there are no breakdowns sent out and the CDs just call those they and the director know they want to see.
From my limited experience I would say that there is much greater tendency to cast the net fairly wide for children's casting than for adults.
