CV writing this week

Decided that I really need to get my CV out to a few agencies. So I'm going to write it over this week as I have a lot of spare time! Any tips or necessities I should include or make sure I do? Should I write anything on it or just a table with my details and name, plays and performances in it? Also, what should I include to really make them want to pick me Togo on their books. How should I tailor each cover letter to the agencies, 1st or 3rd person?
